The Healthy Futures Solution Fund is a multi-year commitment to advance community-led solutions that improve health and well-being in historically under-supported communities. Through strategic funding and partnership, the Fund empowers trusted local organizations to drive measurable, long-term improvements in health outcomes.

Health doesn’t start in the clinic. It starts in schools, homes, and neighborhoods. Improving health outcomes means addressing structural barriers to care, like food security, housing, mental wellness, and behavioral health. By investing in conditions that shape daily life, the Fund helps create healthier futures for under-supported communities.

The Family Van

  • Focus: Mobile community health services that deliver preventive health screenings, health education, and referrals in Greater Boston communities.
  • Impact: In 2025, The Family Van expanded access to preventive care in Greater Boston, increasing mobile van visits by 51% and preventative health screenings by 41% compared to 2024.

Community Servings - Food is Medicine Program

  • Focus: Provides medically tailored meals (MTMs), designed by registered dietitians to support individuals living with serious illnesses such as HIV/AIDS, cancer, and diabetes.
  • Impact: With support from Sanofi, Community Servings reduced its Food Is Medicine program waitlist by one-third, down to approximately 100 patients, and delivered more than 10,000 medically tailored meals to new clients.
